Why did my employer not withhold for my medical premiums?

I pay a small portion of my medical premiums. Less than $2000 annually. I understand that this money is not federally taxable, however, my employer is not withholding SS, medicare, state tax, or local tax on this $2000. I don't understand as I thought this was still earned income subject to these other taxes.
